SEAWORTHY INSURANCE COMPANY AND YOUR PRIVACY
Protection of your private information is a matter of great importance to the Seaworthy Insurance Company and its affiliated companies. While the nature of insurance requires that insurance companies periodically gather personal information about you, Seaworthy Insurance Company recognizes that access to nonpublic personal information concerning your finances and health must be safeguarded. This general notice explains Seaworthy Insurance Company’s overall commitment to privacy with respect to nonpublic personal financial and health information. This notice applies to the insurance product or service you have just obtained from us.

WHY WE COLLECT INFORMATION
We collect certain nonpublic personal information about you that is necessary to review, process or service your requests for products, benefits or other services, and to comply with legal requirements. For example, we may collect nonpublic personal financial and health information to determine eligibility for coverage or benefits under one or more of our products.

THE TYPES OF INFORMATION WE COLLECT
Most of the nonpublic personal information we collect is obtained from you on applications or other forms. Generally we request identification information such as your name, address, phone number, and social security number. Additional information is collected in connection with providing you a product or service or handling claims. We may obtain additional information from our affiliates or others, such as information about your transactions or claims with them. We also may obtain information about you from third parties such as the Department of Motor Vehicles, law enforcement agencies and consumer credit reporting agencies.

HOW WE DISCLOSE YOUR INFORMATION
The nonpublic personal information that we collect, described above, is used to make coverage, service, benefit and other insurance-related decisions. This information is sometimes shared with affiliated or unaffiliated third parties, and as permitted by law, to carry out daily business functions, service your business, or in connection with product offerings. Examples of affiliated third parties with whom we share your information are: financial service providers such as insurance companies, underwriters and agents, and non-financial service providers such as attorneys-in-fact and billing companies. Examples of unaffiliated third parties with whom we share your information are: joint marketing partners, agents, brokers, insurance regulatory authorities, reinsurance companies, claims services and administrators engaged by us to adjust, administer, service or process claims.

Other than as permitted by law, we do not share your nonpublic personal information with unaffiliated third parties without giving you an opportunity to tell us that you do not want us to share the information. We may be required to share your nonpublic personal information to detect and prevent fraud, to protect our legal interests, or to comply with a subpoena, court order, state regulatory directive or other process of law.

YOUR CHOICE WHETHER TO DISCLOSE
If you prefer that we not share your nonpublic personal information with affiliated or unaffiliated third parties, except as otherwise permitted by law, you may direct us not to do so by following the procedure explained in Seaworthy’s Opt Out Form. If you opt out of this information sharing with our affiliates, we may still share with our affiliates your identifying information and information about our transactions and experiences with you.

HOW WE PROTECT YOUR INFORMATION
Seaworthy Insurance Company restricts access to nonpublic personal information to those employees or service providers who need to know the information in order to provide you with products or services. We regularly review our security measures and employee education programs to help protect your nonpublic personal information. We have physical, electronic and procedural safeguards in place. When we share your nonpublic personal information with unaffiliated third parties, we require that they adhere to our standards to keep your information private. This privacy policy continues to apply even when your relationship with Seaworthy Insurance Company has terminated.

INTEGRITY OF YOUR INFORMATION
Seaworthy Insurance Company maintains procedures to ensure that the information we collect is accurate, current and complete. If you have questions about the nonpublic personal information in your file, please write to us with your questions. We will respond within 30 business days after we receive your written request. We will advise you if your nonpersonal public information has been shared with anyone. If we have requested a consumer credit report, we will provide you with the name of the reporting agency.

Please provide your name, address, telephone number and policy number, as well as a copy of your personal identification, such as a driver’s license.

CHANGES TO OUR PRIVACY POLICIES
This notice is effective June 1, 2009. Seaworthy Insurance Company regularly reviews our privacy policies and may change them at any time. We will notify you of any changes as required by law.
